Patient Information

When you need help with scheduling, billing, and paperwork you can count on our office team to help you. They are highly trained and very capable. Most quick questions can be answered over the phone.

Patient Appointments

We only see patients who have appointments. We go to great efforts to make sure that we stay on schedule every day and ask that you do us the same courtesy. If you must cancel an appointment for any reason, please do so a minimum of 48 hours ahead of time to avoid missed appointment fees. We are understanding and sympathetic to the fact that life comes at us fast, and will do our best to work with your emergencies or sicknesses, we simply ask that you do the same.

We will do our very best to accommodate the time and date of your preferred appointment date.

Insurance & Patient Co-payments

It’s never fun to have to do a bunch of guesswork to figure out how much you will owe at the end of a treatment. That’s why we take all the guesswork out of the process and determine the coverage you have available and how much of the bill you will be responsible for.

To assist you in the most convenient way possible we will need you to submit any dental or medical insurance information to us via your New Patient Paperwork. We need this paperwork completed a minimum of two days prior to your scheduled appointment. Remember to keep your coverage information up-to-date with us if anything changes, so that your next appointment will go smoothly.

By getting us this information at least 2 business days ahead of time we are able to obtain a breakdown of your benefits that you or your employer provides. If at all possible, bring a copy of your dental benefits book with you (your carrier ought to be able to provide you with a copy).

If you are ever diagnosed with treatment service, we can provide you with a plan that will give you an estimate of how much of the bill will be covered by your insurance for any given procedure. We can also alert you to what you will be responsible for.

Our professional care is provided to you, the patient, and not your insurance company. The insurance company is responsible only to the patient as a benefit from the employer or individual plan purchase, and the patient is responsible to the doctor.

Financial Policy

Our financial policy is as follows:

•  Payment of account balance is due in full within 20 days of statement date
•  We accept all major credit cards (Visa, MasterCard, American Express, Discover)
•  Payment plans are available through www.CareCredit.com


Appointments & Consultations

We are more than happy to schedule second-opinion consultations with our doctors to discuss possible treatment options diagnosed to you. We believe that it is very important for our patients to be fully committed to their diagnosed treatment plans and we will do everything we can to ensure our patients are well educated on their diagnosis. All referrals to us from other practices and those who have self -referred for a second-opinion will be required to undergo a complete dental examination by our doctors before we can provide a diagnostic opinion.

It would be a big help to have all X-rays and patient dental records from any previous practice sent to us prior to your appointment. Many offices require between one and two weeks for the transfer of dental or medical records.
